Accepted Insurance Plans

These are the insurance plans that are accepted in our office. We encourage you to contact your insurance provider through the customer service number listed on your insurance card to verify that the physician you are scheduled with in our office is in network on your insurance. The list is in no way a guarantee of coverage.

If your insurance plan requires a referral from your primary care doctor, you must contact them and obtain a referral prior to your appointment in our office. Charges due to lack of referral when required will be the patients responsibility.

Patients who do not have insurance coverage must present payment for the visit prior to being seen by physician.

We do not participate with insurance plans for hearing aid coverage specifically.
